Life is unpredictable. Sometimes, situations can arise where a person’s mental or physical capacities leave him or her no longer able make decisions. In those cases, daily decisions must be left in the hands of another trusted person.

In less tragic cases, a person may unable to personally attend to a specific legal affair or piece of business, and wishes to have a designated representative handle the matter on his behalf.

In either case, a Power of Attorney document designates another person to handle the business, financial and legal affairs, either for a specific function or for overall day-to-day needs. This downloadable General Power of Attorney Form, usable in Indiana, authorizes your designated person, or Agent, to act on your behalf in a variety of different situations. Another type of Power of Attorney, called a Durable Power of Attorney, stays in effect even if the Grantor later becomes disabled or incapacitated – the Durable Power of Attorney form is also available on this site.

General Power of Attorney (with specific termination date) – Allows an individual(called the “Principal” or “Grantor”) to authorize someone else (called the “Attorney-in-fact” or “Agent”) to act on his or her behalf. This General Power of Attorney document becomes effective immediately upon signing and stays in effect until a set date, as defined in the document, or in the event of the death, disability or incapacity of the Grantor or if the Grantor revokes the document (whichever occurs first).

This form can be used in Indiana.

Another type of Power of Attorney, called a Durable Power of Attorneys (available on this site as well), stays in effect even if the Grantor later becomes disabled or incapacitated.
